Please download forms, complete and email to schoolforms@stbenedict.com by Thursday, August 1, 2024. Most forms are PDFs that have fillable blanks for your convenience. Some will need to be printed for parent signature. Please scan or upload a clear picture of the form and email.

2024-25 emergency contact form for each student enrolled - (please complete and email) - ONE PER STUDENT
Photo Release Form - ONE PER FAMILY
Family Handbook Agreement – ONE PER FAMILY (handbook available online - updates will be included by July 31)
Narcan/Opioid Notification - All Kindergarten and Transfer Families (not preschool) - ONE PER STUDENT
2024-25 Technology Acceptable Use Policy, Parental Permission and Lease Agreement Forms - K-8 Families ONE PER STUDENT
2024-25 Medical Authorization Form (For Any Students Who Need to have Medicine Administered during the school day - Physician must also complete portion)
Volunteer Forms to be Renewed (for current volunteers - new volunteers, please see below)

Read and sign the Archdiocesan Standards for Behavior.
Complete the Child Abuse and Neglect Tracking Form (CANTS). CLICK HERE for a copy of the CANTS form.
Complete the Volunteer Policies and Guidelines Form. CLICK HERE for a copy of the Volunteer Policies and Guidelines form.
Athletic Forms Required (for grades 4-8) - email to Chris Deakins at cdeakins@stbenedict.com
Submit new athletic medical physical (for new school athletes, rising 4th grade athletes and those whose athletic physicals are more than 1 year old)
Submit new concussion protocol form for the school year

As a reminder, the following items are due to the office by the following dates:
A current medical examination for new preschoolers , kindergarten, 6th grade, and any transferring students are due to Ms. Heather Perez, school nurse, by August 1, 2024. (A medical is not necessary for second-year preschool students.) **Students may NOT start school until we have an updated medical physical on file
New dental examinations for kindergarten, 2nd grade, 6th grade, and any transferring students are due to Ms. Heather Perez, school nurse, by August 1, 2024
New vision exam for incoming Kindergarten and any transferring students are due to Ms. Heather Perez, school nurse, by August 1, 2024
THIS YEAR: Homeroom Assignments (PK-5) and Middle School Schedules will be available the week of August 12, 2024, to families who have returned all their forms. Parents/guardians will be able to access homeroom assignments and middle school schedules via FACTS. These items will not be mailed or printed.

The elementary and middle school office is open Monday through Thursday from 9AM to 12PM. The preschool office is open M-F from 8AM to 5PM until August 2nd.
