Revised Permission Slip - please review

The Office of Catholic Schools revised the required field trip permission slip and we are no longer able to use the previous permission slip. I have received some feedback from parents that they are not comfortable with some of the language in the permission slip.

Please know that we are required to use this permission slip. I have included a generic permission slip for parents to review in advance.

Parents/guardians are not able to cross out/redact any part of the permission slip. The permission slip needs to be returned as is. If a parent/guardian is not choosing to sign the form and send their child on the field trip, then please make plans to pick up your child from school during the duration of the field trip.

I would like to assure parents that the changes are coming from the Archdiocese of Chicago. The changes have not happened because of any event or issue stemming from a field trip sponsored by SBPS. I am not aware that the changes are a result of an accident/issue with any other Chicago Catholic school.

Finally, I want to assure parents that we always take safety very seriously when we plan an event or trip. Regardless of what the permission slip says, it is our expectation that all teachers, chaperones and students follow all safety rules at all times. We hope parents decide to send their children on these great field trips and continue to have confidence in us.
