Homeroom Assignments (PK-5) and Middle School Schedules will be available the week of August 11, 2026, to families who have returned all their forms by Thursday, July 30th. Parents/guardians will be able to access homeroom assignments and middle school schedules via FACTS. These items will not be mailed or printed.

Medical Forms
Due July 30. Email to Nurse Perez (HPerez@stbenedict.com)
Required for: new preschool, kindergarten, 6th grade, or any transferring student
Medical information for kindergarten — if entering kindergarten
Medical information for second grade — if entering 2nd grade — dental exam required
Medical information for sixth grade — if entering 6th grade — dental exam required
Medical information for students transferring to SBPS, grades 1-8 — if transferring
Medical examination form — generic form, use if a grade-specific one above doesn't apply
Dental examination form — required for new kindergarten, 2nd, and 6th grade
Vision examination form — required for new kindergarten and transferring students
For any family — reference and care plans
